Lifting Session:       Saturday January 7, 2005 at 9:00 a.m.
                                 Mandatory Rules Clinic at 8:30 a.m.

Sanction/Rules:      Sanctioned by the American Powerlifting Federation (APF).
                                Check worldpowerliftingcongress.com for information and rulebook.
                                NOTE:  The AAPF division is tested for anabolic substances.
                                One piece lifting suit required for both lifts.

Weigh-ins:             January 6, 2006 from 9 a.m. to 12 noon, 7 p.m. to 8 p.m.
                               January 7, 2006 from 7 a.m. to 8 a.m.
                               At the Velocity Sports Performance -  Early weigh-ins highly encouraged

Eligibility:              Open to all registered APF/AAPF lifters.  Lifters may register at the meet before weigh-in.
                                Registration is $30 per card, $40 for both APF and AAPF, $15 for students.

Entry Fee:            A fee of $50 must accompany this entry form, and be to the meet director no later than
                              Friday December 30, 2005 - Postmark Entry by Wednesday December 28, 2005.

Awards:               Trophies will be given to the first three places in all Men and Women divisions.
                              Best Lifter awards will be given based on the content of the entries, and will be
                              decided according to the Glossbrenner Formula.

In consideration of the acceptance of this entry thereby for myself, my heirs, executors, and administrators waive and release any and all claims and damages I may have against the APF/AAPF, Eric Stone, Velocity Sports Performance, their representatives, successors, and assigns for any and all injuries or bodily harm that I may suffer while competing at the 2005 APF/AAPF Chicago Summer Bash II.  I attest and verify that I have full knowledge of the risks involved with my participation in this event, and that to the best of my knowledge I am physically fit and able to participate in this event.  I understand there is random drug testing by urinalysis on a minimum of 10% of all AAPF lifters.  AAPF/AWPC policy permanently bans any lifter testing positive for a banned substance.
